At your visit you be required to complete, sign and return medical Registration Forms.  These forms must be updated yearly for your medical chart.  For your convenience, the Registration Forms are available to download, complete and bring with you to your visit.

Billing and Payment Reminders (for visits)

Financial responsibility will vary from patient to patient and will depend on medical services provided. With all our patients, we make every effort to inform them of their estimated financial responsibility. We accept cash and all major credit cards for payment.

Bring your insurance card to every visit.

  1. For insured patients: Check with your insurance carrier to make sure that Rene I. Luna, M.D., P.A. is in your network. It is important to bring your insurance card to your visit to ensure we have the correct insurance provider, insurance plan, member ID and billing address to send your claims.
    In order for your insurance to be billed for your medical care, our office will verify your active status for each date of service with your insurance provider. It is essential you notify our office immediately with any insurance change to avoid accounting errors. We will not retro bill any insurance for prior dates of service.
    Our office works diligently at every visit with your insurance provider to obtain the most accurate information regarding your benefits to better inform you of your estimated financial responsibility. Insurance providers only provide our office with estimates based on your specific plan and never guarantee coverage of the patient or reimbursement to the provider.
    We make every effort to ensure the financial estimate provided by your insurance provider is as accurate as possible by ensuring we ask for a detailed reason for your visit to ensure we ask your provider for the proper authorizations that may be needed for your care.
    Our lab is a third party company located in our office for your convenience. Please note, all laboratory services are billed by the lab directly to your insurance company. Our office does not collect payment for laboratory services for insured patients.
    At every visit, you are required to pay co-payments, deductibles, co-insurance or any “non-covered” service under the terms of your insurance plan.
  1. For uninsured patients: It is important you contact our office directly to be provided an estimate for medical services. We make every effort to ensure the financial estimate provided to you is as accurate as possible by ensuring we ask for a detailed reason for your visit to estimate charges for all medical services.
    Our lab is a third party company located in our office for your convenience. Please note, for uninsured patients our office will collect for laboratory services as we pass along our discounted rate from the lab to our patients. Payment is required at the time of your visit for all medical services.

Your prompt payment enables us to continue providing the highest quality service at a reasonable price.


Know Your Contract & Company

The more you know about your own plan, the greater the benefit you can enjoy.  If you have questions about your insurance coverage or wish to raise a concern about the manner in which your insurance company paid our bill, please call your insurance company directly. They need your input in order to improve their service.

Payments

Many health plans require a co-pay for office services. Please be prepared to pay your co-payment at each visit. If you do not have insurance, please be prepared to pay for the cost of the visit at the time of your appointment. We accept Visa and Mastercard credit cards and Visa/Mastercard debit cards for payment.

Other Fees

You will be responsible for all additional fees incurred by the clinic associated with your care or account. For example:

  • Non-covered or non-medically necessary lab tests,
  • Copay, co-insurance and/or deductible for when other procedures are necessary during an annual exam (anything not included in an annual well-woman exam), and
  • Any extra procedures needed during your visit that were not quoted before your visit.
